Maude Muriel was the daughter of Samuel Albert Hazen and Emily Lanning. Her parents were born near Oak Forest, Franklin County, Indiana.
Following the marriage of her parents in Indiana, they set out "west" in a covered wagon, stopping over in Pender, Nebraska for several years where Maude was born. From there the family proceeded on to Oklahoma.
On 3 Feb 1913 Maude married Phillip Samuel Harold Johnson in Newkirk, Kay, Oklahoma, the county seat.
Note:

The death certificate shows a date of birth of 9/1/1891 while some say 1889.
