March 8, 1976 - December 28, 2021

Chad Jason Smith, resident of Oklahoma City, passed away on Tuesday, December 28, 2021 after a battle with Amyotrophic Lateral Sclerosis (ALS), more commonly known as Lou Gehrig's disease. He was 45 years old. Chad was born on March 8th, 1976 in Enid, OK to his parents, Charles Raymond Smith and Lynda Kay (Bottrell) Smith.

Chad graduated from Ponca City High School in 1995. He went on to further his education by receiving an Associates degree from Northern Oklahoma College in 1997, a Bachelors degree from the University of Central Oklahoma in 2004, and finally a Masters degree from Oklahoma City University in 2006.

Before his ALS prevented him from some of his activities, Chad enjoyed working out at the gym several days a week. He also helped others lead a healthy lifestyle by being a personal trainer and health advocate for eating healthy and maintaining a positive mental attitude.

Chad earned his CDL license and became a Warehouse Supervisor/Chemical Mixer/Delivery Driver for Dwight D Smith Company and then Ram Products. His favorite responsibility at his job was training others for OSHA, GHS and DOT regulations and guidelines for hazardous materials. He was very thorough and loved every minute of it.

Survivors include his father, Charles Smith of Ponca City, and sister, Lori Smith and fiance Pat Hope of Oklahoma City.

Chad was preceded in death his mother, Lynda Smith, and sister, Michelle Smith.

A graveside ceremony for Chad will be held at 10:00 am on Monday, January 3, 2022 at Newkirk Cemetery. A reception will follow immediately after the graveside at First Christian Church, 210 N 5th St, Ponca City, OK 74601.

Arrangements are under the direction of Trout Funeral Home & Crematory in Ponca City.
