Hazel Ann Bentley
April 7, 1938 ~ February 12, 2019


Hazel Bentley formally of Anadarko, was called home on Tuesday February 12, 2019 due to on going health complications. Hazel was born on April 7, 1938 in Binger, Oklahoma to Elizabeth Edge and Thomas Bentley.

Hazel is preceded in death by both parents, one sister Dian Sparks, two brothers Tim and Tommy Bentley, and one son Stanley Chaukenatho.

She is survived by one sister Charlotte Bentley of OKC, OK; one daughter Patricia Vermillion of Elk City, OK; three sons Teddy Chaukenatho of Anadarko, OK, Danny Haggard of Elk City, OK and Jesse Querdibitty of Carter, OK. Granddaughters Nikkita Querdibitty of Anadarko, OK, Korree Gomez of Chickasha, OK; grandsons Dustin Durland of Stigler, OK, James Durland of Elk City, OK, J.L. Burford of Gastonia, NC, Winston Burford of Anadarko, OK, Devin Burford of Anadarko, OK; as well as many great grandchildren, family and friends whom she loved very much.

Prayer services will be Monday, February 18, 2019 at 7:00pm at the Smith Funeral Home Chapel. Funeral services will be Tuesday, February 19, 2019 at 10:00am at the Smith Funeral Home Chapel.
