Millard died on October 6, 1919 at 4:28 p.m., at the age of 6 months and 2 days. He was listed as a white male. Cause of death was listed as obstructed bowels, (per report), the child was operated on before death.
T. S. Williams M.D., was the medical doctor in charge. He states that he attended the deceased for 48 hours, and that he saw him alive on October 5, 1919.

Jim Mattox of Stilwell was the informant, W. C. Lee was the registrar, report was filed on October 6, 1919.
He was buried in the Chuculate Cemetery on October 8, 1919.
