Services for Jay Lee Perkins, 52, of Ada are 2:00 p.m. Friday, January 6, 2023 at Trinity Baptist Church, Rev. David Gray will officiate. Burial will follow at Rosedale Cemetery. The Chickasaw Nation Honor Guard will conduct military honors at the cemetery. The family will receive friends from 5:00 – 7:00 p.m. Thursday, January 5, at Criswell Funeral Home.

Mr. Perkins passed away Sunday, January 1, 2023 at an Oklahoma City hospital. He was born July 7, 1970 at Tishomingo, OK to Larry and Cathy Moore Perkins.

Mr. Perkins worked for the Commerce Department of the Chickasaw Nation as an Inventory Control Coordinator and he had proudly served in the U. S. Navy.

Survivors include his wife, Gina, of the home; his mother, Cathy Ryan of Asher; four sons, Daniel Perkins and wife Marilyn of Ada, Gabriel Perkins of Satsuma, AL, Devin Cothren and wife Samantha of Stratford and Trenton Cothren of Ada; two daughters, Hannah Perkins of Sacramento, CA and Rebekah Wright and husband Adam of Yorktown, VA; a brother, Eric Perkins of Indianapolis, IN; sister, Amy Perkins; two aunts, Jackie Ferguson and Gail Moore; his father-in-law, Bob Adams; his grandmother, Arlene Franklin; three grandchildren, Ethan Ward, Bryar Cothren and SaigLee Cothren; and many nephews and cousins.

He was preceded in death by his father, Larry Perkins.

Bearers will be Adam Wright, Daniel Perkins, David Cavener, Trenton Cothren, Devin Cothren and Zach Perkins.
