Perry – Dwayne D. Kukuk, 21, Hennessey, died Thursday in an Oklahoma City hospital as a result of injuries sustained in a one car accident September 16. His service will be 2 PM today in the Christ Lutheran Church at Perry with the Rev. Walter Wehmeier officiating.

Burial will be in Richburg Cemetery at Perry under the direction of Parker Funeral Home, Perry.

Kukuk was born September 5, 1957, in Perry, and attended the Perry schools. He had been a driver for Western Tank Company at Hennessey.

Survivors include his mother, Mrs. Roger Lewis, Perry; his father, David Kukuk, Perry; two sisters, Kathy Gayle Estes and Brenda Lewis, both of Perry; three brothers, Terry, Gary and Danny Kukuk, all of Perry; maternal grandparents, Mr. and Mrs. Fred Karcher, Perry; paternal grandparents, Mr. and Mrs. Clarence Kukuk, Perry; and great – grandmother, Mrs. Emma Pagel, Perry.
